It's unlikely, if only because Elsa, her cousin, wasn't sent anywhere but became a model?
But this will be one of the supporting versions-theories.
I would also add that Nika is probably also from some destroyed family like William.

I'm pretty sure I remember Ocean stating on discord Bella's sister isn't a twin. I think she's the older one. I'm not going to try and search for the details though, someone ekse can find it faster I'm sure

Or...maybe nothing is as nefarious as we're making it out to be. Maybe in SG, the Mafia is the time we waited between updates along the way.
Maybe Bella's identical not-twin just drank still water, got a brain infection and fell into a coma and she's just waiting for the MC's magic schlong to wake up.
Summer? Maybe she just caved into her parents pressure not to date a loser and ghosted the MC after the family moved.
The MC's accident? He chased the car as Summer and her family were leaving on a bicycle, swerved onto incoming trafic and got run over by another vehicle. Blood, collapsed lung, hospital yada yada.
The MC's "criminal record" as a minor? He stole and totaled the damn bicycle he used to give chase. Dun dun dun!

"MC's accident" is because fights. It was explained in the game.
His lack of self-control make him do the "bad thing". Summer's disappearance only make him Depressed. But his lack of self-control caused by Cheeto.
The answer explained when you take this path.

Nobody knows what this game is about. I used to think that SG would be an interesting crime drama related to the search for Summer, but after 6 years of development and a bunch of reworks that changed the game beyond recognition, I'm not so sure anymore. Maybe I'm wrong, but it seems to me that the author hasn't decided on the plot of SG yet.

To take everything people love and then mix it into something which feels fresh and unique. I remember when I first got into Lost it soon ended up becoming something else. The popularity of the first season made them stretch it into something so absurd it became the biggest waste of time ever.

I hope she does. Maybe that´s why Bella became how she is. Or maybe she was always like that, but I´d like to believe there is at least a "redeemable" reason.The picture of the two of them is whta makes Nika ask Bella about her sister the first time he's over her house to study. Because of her reaction, and the way Ayua reacts about Nika asking about Bella's sister, it's safe to assume something bad happened.
Because she appears to be in home care, and there's all those missed presents, it's be really weird if it was a stranger ...or former roommate.
And there's enough life support gear there, to consider that if she's in home care, they've set up for the long term. Bella talking to her knowing show won't get a response, the unopened presents etc, wishing she could see Nika's eye's someday, all suggest she's not waking up.

Ah, but it's 100% certain that Bella became like that after her sister's “coma.” Just like Nika with Summer's disappearance. It's almost the main theme of the game, when you think about it.
Otherwise, Nojiko has ties to the Zane family, at least to Katie, and Nami received one of Leia's earrings, so... Either Nika and Nami's biological parents were connected to them, or it's Nojiko. It would be funny if the Zane family were watching over this sibling group from afar.
