Sorry, not even close to my area of expertise
Maybe try to whitelist it in windows security?

Also, the "modded incest patch" doesn't include 4.5 content, your "sister" is back as a "roomie" during 4.5 content

Are you sure it doesn't work, this comment says to keep playing to make sure it isn't working.

Also, per instructions, did you use UNREN and extract the RPA packages?

I suspect, it's just an old mod. Because the latest is version 4, and the game is 4.5.

there are lots of in game changes i.e. script changes, as such most old mods won't work.

If u use the mod and start over it asks if you wanna skip to the beta scenes so it will most likely skip the new content
