That's nothing. The time between v20.0 and v21.0 was 5 and a half years (2019-may 2024).

if you have finished the old 20.16 version then no. they still keep updating the old stuff. One might now argue they added new scenes, but none of them are worth replaying right now. The best course is to not pay their patreon, return in 4 years, and maybe, and that's a big maybe, they will have reworked the whole game so that their might actually be something that could be considered as new content.
i've seen that too, but i think it's just fanmade. the only official "futa" in the game is just Eve, i think

It's not a new engine. It's the same engine - Ren'Py - as it was always stated it was. The code for the game in the engine was shit. That is what is getting updated. Not the engine, itself.
A few new scenes - but overall not much content., Come back in a couple of years.

I hope DC delays the prequel of the pre-tech pre-patch until 2099 so he can get Miguel O'Hara himself to deliver the updates to the patrons with a well-penned "sorry I'm late" letter.

i think you can just copy and paste it in the game folder... try look the folder full of .rpy files.
that should do it.
