Doing my first playthrough of this game and I'm currently in chapter 1. If I want the harem ending with the most girls possible, do I select a open relationship with Nicole or do I break up with her?

Nicole isn't necessary to have harem ending. But harem here is a little different... See for Yourself and gather as much points with girls as You can.
There is a walkthrough available which is very helpful in determining how many points needed with certain women, and also lays out the possible endings

she's more in on 3some endings so your call. also harem ending is on extra endings.

Have you checked the PDF walkthrough to see what you're doing wrong? That's pretty good with the dos and donts for each path.
You do have to find the documents. You can't get Shortcake on the bad ending.

I'd read or follow the PDF, It will give you all the things you need, like how many points and where to get the points. Etc.
It's in the OP if you don't have it.

She's Hirami, she was Roshi's nurse. You can have some fun with her after Roshi dies.

she appears when rishi gets home nurse but sex with her appears only after rishi is dead and isn't on rishi's daughter's path. she has only few appearances and isn't li so not that big loss.

You only get her if you make a promise to Roshi in Chapter 1.
Make sure you are not on Yuki's path at Roshi's funeral and Hirami will talk to you.
Go to visit her later and your choices will give you three different options (try them all).
And then she'll ask you later to her massage parlor for another scene.
(There good scenes, well worth checking out).
