IIRC, Liz and Kyouko's fates as far as we know are still written down in the datamined perks so they should be firmly part of the main story independently of the endings, is just the other girls who are up in the air and may change depending on the different endings, unless Fujino makes a completely 360º ending out of nowhere, which is very doubtful XP

So I can only think of two reasons why there's an open condom.
Either it's not a very logical action by Fujino and he put a condom in there just to make the scene more lewd, because authors are very fond of adding condoms scattered all over the place.
Or they don't have any pills, and Inoda uses her usual tricks and persuasion to get Asuna to try it without them, promising to pull out in time or something like that. But in the middle of sex, when Asuna loses her head as usual, Inoda forgets all his promises and starts cumming furiously in Asuna's pussy.

As to why they are using condoms when they can use birth control pills. They could get birth control pills, probably from the same place they got the condoms, the hotel, but Asuna still don't want to have raw sex because she does not want to cross that line with Inoda. It would make more sense if Fujino used Asuna's pregnancy in FOG to make her want a child in the real world. But this whole situation with Yui seemed to be necessary only for Inoda to turn out to be a father(obviously, he didn't really want this and, obviously, he would be a bad father). It feels like Fujino just ignored the excellent transition to the pregnancy fetish in the real world and just used Yui's ritual as an excuse to "change memory." Yui was just born and it didn't change anything in Asuna's life and behavior at all (changing her memory is a outside intervention, so I don't take this event into account in this situation). It's even surprising that Inoda constantly talks about wanting a child with Asuna, but Asuna's reaction is very mediocre. No consent, no clear refusal. Although she should realize how much this will change in her life. But it's like Asuna doesn't care. Even after Yui was born with Inoda as her father.
Regarding condoms and birth control pills, there was a dialogue in some scene with Kirito peeping that they couldn't find pills at the hotel, so they would use condoms. If I remember correctly.
So if pills had suddenly appeared, it would have been a plot error.

I am pretty sure I remember Asuna exploring her house the next morning, finding the extra key and wondering where it came from.
Even if it did broke after one use, that "can only be used a few times" detail turned out to be pointless. Which is just bad story telling.
That would be extremely lame. Inoda has been getting what wants from Asuna through pleading, manipulation, and plot contrivance. Down right lying is just lame at this point.
