It hasn't been announced yet what Fujino's next game will be based on. Everyone is just guessing or hoping it is Naruto. Fujino did make this picture a while back but it doesn't mean he will do Naruto...

Fairy Tail would be cool, Fire Force or even My Hero Academia seems legit too.

Yesn't, technically you DO get some small stuff that's different only in low prurience (Like one reaction from Liz to certain words said by Kirito for example), but essentially, yeah, it just lets you do/see more stuff in general. Also, as mentioned, a good thing of the game is that you can choose to start at almost any point of its timeline and you will get questions before doing so if you do it late into the game to determine if you have been doing stuff to raise your prurience or not, so even if your first full playthrough was with one you can easily see the other reactions without having to replay the whole thing :O

Just downloaded the game, bit confused on where to start, whats the first chapter/section to start?

Are you perhaps using old save files? It's been repeatedly stated you never do that, you erase everything with every new update, older saves corrupt the game as they become obsolete with a new update :O

I shouldn't be, I downloaded the game with the hotfix, added the hotfix in and started the game from scratch
