Done when portal quest is started (you can refer to wikiK. You can buy a ring gag, upgrade it from a blacksmith or a general shop in Town of Monsters to this oral fixation, then you camp and rest.
I know about the portal upgrade, got the Mouth Fiend encounter already
I was asking about flavor and oral fixation, these have nothing to do with that particular encounter

purely from reading decompiled game code:
Oral Fixation works as an alternative to Mouth Maniac perk, you just get the same oral focused encounters as if you had that perk on lvl3. (Harpy & Town Square Lewd)
As for Flavor, I am not really sure the code doesn't do much with it from I can see, theres an animation that gets played on equip but thats all I can see here tbh, then again decompiling isn't perfect either.
Good gods, I last tried to play this game years back, one of the clear examples of a smutgame where the whole "game" part comes at the expense and lessens the "smut" part.
Is it still like that? Or is there a "story" mode that lets you just pick the options you want to see.

Dunno how long ago you've played, i've been here few years by now and i've thought the game has a good balance of smut and gameplay, it's not just complete porn plastered over a hastily put together system.
But hey, to each their own and whatnot, give it a spin and judge it yourself, i'm sure there's been lots of content added since you last played.
I mean there's the encounter option if you turn on cheats so you can select which enemy/scenario you want to see. If you're asking if there's options to skip combat/skill checks then no, closest you could do is save edit your character to have overpowered gear before you start.

My game has a bug when I enter into a fight against the vampires, there is a chance that we both go in the wrestling mode and just stuck there forever, I only have the "grapple, reversal and rest" options, and you can only spam for eternity, can't leave the fight, can't attack, nothing.

Nope. It's not. Your personal download might be corrupted or incomplete. You might be doing something wrong with the JAR once you get it on your computer. But the JAR is not corrupted before it touches your computer.
