After a few playthroughs this is what I got the last time.

I think all of these happened because of poorly defined routs in game because at latter chapters where she should not sleep with someone she does and keeps it secret. Is the main reason we're getting revamped version of the game, so stuff like that does not happened.
I went through the script for CH1R and there are no routes defined yet, just labels for future interactions. In the legacy TAC, the variables only come into play at Chapter 4 or 5. Maybe it will be the same.

One reason might be because, Anne probably feels less in control of her life, the more she tells hubby about her experiences. Another reason could be, selfishness. Maybe she feels some intimacy towards THAT lover and don't want to spoil the memory by having it turned into a cheap thrill for hubby. Putting aside the eccentricities of the paths in the previous version of the game for the sake of argument, the player's choices do affect Anne's actions and how far she's willing to go. I've been saying for a while that the sort of core philosophy of the game was slowly but inexorably revealed to be "be careful what you wish for" rather than the uncomplicated sexual romp most players (including me) initially took it to be.
The story begins with the MC pushing Anne pretty hard and not always showing much empathy, or even concern, for what she wants. When she resists or states her own contradictory preferences — and on occasion she does — he outright ignores her. He can (if the player chooses) also repeatedly cheat on her, and despite her making it clear on multiple occasions that she's not particularly okay with this — especially when he does it without her knowledge or consent — he never changes. It's completely rational for Anne to conclude that she should look out for her own interests and take control of her sexual exploits, and it's also completely understandable if her willingness to view him as an equal partner in those exploits erodes as a result.
A properly constructed path system in a game that offers choices between swinging, hotwifing, and cheating would only have this occur on routes where the MC has failed to consider her feelings, cheated on her, encouraged her to cheat on him, or otherwise given her reasons to lose respect for him. (With the major caveat that @Mircom3D may have a different idea about how the game's going to end, no matter what path you're on.) Right now (in the original game), Anne will eventually cheat no matter what choices the player makes. Just how much she cheats, and with who — and just how much the MC cheats — remains up to the player.

We'll have to see how it goes with the retooling of the choices, it will be nice to have it more concise. One thing I would really like to see, I know I mentioned probably dozens and dozens of pages ago, is the "I'm too tired" options having an effect, they're basically just a skip scene button right now, possibly that's the intent. Would be an interesting way of Anne possibly taking things into her own hands if you keep not having sex with her(or ignoring her in general) by always choosing the various forms of I'm too tired, not there when she's packing to go to Kenny's, etc.

There's a lot that could be done (but wasn't in the original) with the early chapters, along the lines you indicate and elsewhere. Kenny's choice to film her without her consent (and to share those pictures with the MC) could be made significant; at the moment, she complains about it but there are no consequences. Additionally, by the time we get to the vacation, the MC's choice to watch (or not), to go have his own fun, or — and this would require reworking the scenes — participate could show, rather than "tell," the couple entering into a swinging vs. a hotwife/cuckold/cheating relationship.
I have no idea if that's part of the plan, though. I kinda suspect it's not.
