Ciara, she's a woman (no dick) and also she’s a vampire-metamorph, she can temporarily change/adjust or grow certain body attributes. For example, she extends her tongue while giving a BJ to MC, and in Jenny’s flashbacks at the Violet Lips Club she briefly grows a dick. Though, as far as I know, that stuff is (or rather will be) optional when it comes to MC’s interactions with her.
