It will be progressive, so probably starting out it would just be a quirky, air-headed personality change and then lean towards a more corrupted "addicted to sex and can't help but get on their knees at the slightest suggestion" side as it progresses. I would say I enjoy the braindead sex-fiend aspect of the latter more, so that would be more of the focus but if someone wanted than they could definitely just choose not to progress and keep their PC just a little ditsy.

It's my main goal of this game to try to always have the PC interact in both perspectives D/s-wise, or at least have both perspectives have the same amount of content (i.e not all encounters will have a clear, diverging dom/sub path, but instead there'd be an equal amount of encounters.) It's easy to lean into the submissive PC side of things with a lot of the themes in the game though, as, like you said, that's typically the formula/what is associated with this genre of game, but my hope is to have the final version of the game as close to a 1:1 ratio as I can.Is it planned to have both dom/sub variants?

You bring up a good point in the voting process; it'd be unfair for someone to vote for, as an example, foot content assuming the PC would be the one dining on toes, but then in the new version it's the PC's feet being the buffet. In future polls I'll probably put in parentheses what perspective the theme would be if it wins.
TL;DR:
The game is about both sides for everything. MC can get feminized and feminize others, can get cucked and cuck others, can get hypnotized and hypnotize others, etc etc.

A main goal is to have as close to 50/50 dominant to submissive content in the game. Many encounters will have both approaches, but there will be some strictly dominant and strictly submissive routes.

Solid start! I really enjoyed the first sex scene and all the different ways it went (yes I went back to experience them all lol). The stuff after it felt a little unstructured, but I'm sure other people found the freedom exciting, so not really a complaint. Excited to see where you go with this, and find out what the contest is about this year (I assume something lewd but we do have sword and stealth skills available, so I imagine it could be something else involved!)

Haven't checked on this game in a while, glad it wasn't abandoned. Anyway, mind if I ask where the CBT stuff is? Can't find any of it, though I didn't check thoroughly. Is it part of the main thing, a random encounter, or something else?

The currently implemented scenes are in the prologue, you can either choose to Dom or Sub to the tavern stranger. For the sub route, choose to try to break away after getting second thoughts, then choose "Fuck you" (You'll need NonCon enabled for the rough submissive route, Bondage enabled to get tied up, and of course CBT enabled to see the scene.); as for the Dom route, choose "be forceful and rough" and then "torture them", then it will be one of the options. (NonCon enabled to be rough, and CBT enabled to see the option.)
I loaded up an old save, then got stuck after I went to sleep in the Inn because the box had no Wake Up option to click on. Oh well, started a new game. Then I got stuck in the adventurer's guild after agreeing to be a footstool, the next box was empty.
Some of the character customization is kinda abstract to me. How big are huge breasts? Does thick mean fat or thicc? Why is their no ass size?

Alright, so. When doing the adventurer guild stuff, and going down the sub route. Once you pick the "act as a footstool' you end up eventually going to the "Follow him in shame" link. Which heads to a dead end:

I also noticed that by default, your traveller's clothes aren't equipped automatically. So when you for example go pick up the tavern outfit from the clothes stall, it just reads $CurrentOutfit instead of anything else.
