I thought it depends on what path you're on. If Luke is cheating and having some fun with Sandra (the nurse) Dominic might go further with Kathryn if she lets him, while thinking he's Luke. However, on looking at the files, unless I'm missing something, there are a few renders that aren't called in the script, so I don't know if this sequence is possible to get, meaning in every instance Kathryn is the one who first realises it's not Luke:
There's a big difference when she's in control of the situation versus when she is not. If you made the choice for her to pleasure him on the train, she knows what she's doing and has made her choice. In the doctor's she is taken by surprise as she didn't know that he was there/it was him and he was essentially about to rape her, so it's not surprising that she was pissed off.

They are in completely different circumstances. When Kathryn is in control, it's a safer feeling, whereas when she is blindsided at the doctor's, she's not in control so it's not a safe environment and she lashes out.
Dominic doesn't comes across as someone who has that sort of control. He's clearly a bit delusional regarding Kathryn, which her fooling around with him previously has (unfortunately for her) fed that delusion.
