Are you sure you have a legit version? She's supposed to have an icon with her golden hair look once Cerys shows you the illusion of her, but that icon gets updated to her new dark look once she shows up at the manor later.
I have the version that is available here. Maybe the walkthrough mod broke something?
Either way, its not such a big deal. I just found strange that she was the only one who didn't have a icon.

i think the icons/character entry only shows up if you make the "correct" choices. i could be mistakes though.

Orlaith profile gets question mark around first time she mentioned in dialogue (couch talk with Cerys and Luther) and fully unlocked after scene of meeting her for first time (back in chapter 1)
are you using old saves? dev keep reworking scripts every update, it possible you got past that section on older update before variable was added
either way, tested on fresh start + wt mod - profile gets updated without problems
