As I said, I don't know if the mod is the problem, because it doesn't replace anything in the game.
I imagine in the last Ep, the MC takes a grip of his hair on top of the head and with a short move, he removes the mask, revealing white hair, yellow eyes and a big scar in the face. This would explain everything.

Without seeing the errors it's impossible to say for sure, but if the game has been modded it's 99% safe to say the mod is the problem since neither I nor any of my testers had a single error message let alone tons of them.I've just started the game again, and I've got a lot of errors in chapter 1 at the moment.
