Roadmap suggestions
NSFW content ─ it's all a matter of storytelling and not of simply satisfy some people's fantasies/pervertions and all of these make sense in relation with gameplay and, priorly, with human nature... this is definitely the inspirational area in which more content can be added enriching the game and keeping the narrative and the game alive:
- Self-erotism?
* ─ a human natural instinct and necessity, as obvious as it sounds.
- Sex with toys/sexmachines?

─ perfectly suits certain situations involving "bad" encounters, specially in groups... rape is not my cup of tea at all but we're in a survival game and having that is just depitching reality, disgusting to me but still realistic specially.
- Sex with parasites?


─ definitely not my cup of tea and, in my opinion, totally unrealistic and a real nonsense with no natural logic.
- Sex with zombies/mutants whatever you call them?

─ not my cup of tea either but still a logic behaviour bcause we really cannot know for sure that primal instinct wouldn't still belong to
- Sex [consensual] with rare random human encounters?
─ because might sound vanilla but it's still live what we're talking about!
- Sex with tamed dogs

─ I know wasn't planned but there are always compromises to be done. Dogs are the only true friends the protagonist has in a survival environment so it's not just "sex" if you give enough depth to the character but necessary love-making at some point... it's a morality stretch, I know, but we're talking about a post-apocalyptic reality and environment where morality severily shifted inevitably and dogs packs have their own rules which in this case can easily involve the protagonist wherever she's the alpha or not.
- Sex with horse
─ cannot apply the previous same logic but oral sex, for example, definitely becomes a logical development in the character's survival because of proteins and in case of thirst... another little stretch perhaps, because I must admit... it's a fetish of mine I enjoy at least as much as the previous point and I admit that, but still much more acceptable then cannibalism, for example, which we know has historically become a human conseguence in extreme survival conditions and by the way I've understood you have found an intelligent way to include that in your game... not my cup of tea again, at all, but at least I can see that's an essential dynamic in your game/story narrative core as much as "amputations as punishment for crimes" and the "misuse" of it by a corrupted imperial society are a core-part of my own game.
Gameplay/Mechanics:

─ If you cannot accept compromises because of legal reasons, give people still some freedom for self-made content. Since UE is free to use just make the related assets ready for additional work to the modders, for example by including possibility to add animations or new interactions with NPCs/animals or whatever breathes/eat/drinks/piss/whatever. I know would take some time to add but there ae already tools to help you with that.
Just a few considerations about NSFW content and why mods are fundamental in
adult content friendly games:
Games ike Skyrim and Fallout 4 can offer any type of adult content which would have killed their franchise and Bethesda would have gone through a legal hell if they developed such content themselves. I could extend the example with other games and developers but you got my point, right? That's why and how important mods are.
It's saying the obvious mentioning how important the modding community has been for FO4 and, even more, Skyrim... those games would have gone dead pretty quick by bugs, glitches and probably even lack of fresh new content.
You are a solo dev... making your game mods-friendly can help you to get rid of developmental stress and make you focus on what you had in your initial plans while engaging current and new players and give them all the reasons and tools to keep their attention up, make them feel invested and encourage them to support you even more as much as I believe your game does deserve after all.
I know by experience how though is being a solo dev despite UE huge potential and intuitive-learning curve so when reading your frustrations you previously shared with us ─ was appreciated at least by me ─ I could feel them and relate to them big time so I just want to encourage you, just like I try to encourage myself developing my own game.
