I tried to do it in some scenes, but it not looks like bulge, just stomach size increasing. I want to do bulge yes, in some scenes it looks like necessary.

No pregnancy. I wll do stomach deformation events, mb some eggs involved(related to upcomings vents gamepley). But not the pregnancy itself or berth events.
Infection just simple mechanics referring to original character, like when it reach some value, she need medical attention.
