It auto-saves whenever you enter a room or a new floor. You heal by getting health orbs (green round things) that appear after you open chests, as well as through a few other methods (potions, etc.) And "getting things done" uh... I honestly have no clue what you mean by that.

Just go to the hub and equip the big two-handed sword. That shit's overpowered as hell, game basically plays itself once you get some good upgrades that synergize with it.

Holiness works unstable. You never know if you have enough holiness points to enter the holiness room when the transition between levels appears. Also, there are a lot of corruption items, especially compared to the holiness items, of which there are only 4. This is very unbalanced. It only happens if you kill those whip guys before they spawn the enemy in, and you can just go back to the menu and continue the game to fix the bug.

I haven't really noticed any such pattern...it kind of takes the immersion out of it anyway, especially considering it's the final stage of the game.
Agree on the item bits. I would also add a holiness or corruption symbol for each item, prior to picking it up. So the player knows what reputation each item gives.
