Get rid of other enemies on the screen first so the cock monster has nothing to waste time on healing. Preferably in a room with the swinging axe so you can still get easily knocked down.

Nope, if you fight the enemy in the room and back to the title and continue only the enemies and drop that are reset (not randomize). This work on treasure chest too except the mimic monster chest and the one in special area with campfire (where you see the flashback). Basically if you open the chest and the content is pitiful you can reset it by back to title as long as you don't go to the next room

Meanwhile if you go to the next level or floor, all the rooms in the next level/floor will be randomized but once you arrived, all rooms will be fixed and you cannot change this, the room with enemies will always have enemies, the merchant room will always have merchant and his wares is fixed too. That's why it's recommended your back up save is in the warp/teleport room (the one says Press E to the next floor). If you go to the next floor and all rooms didn't have something good like there is no relic room (the one you open using key), overwrite your save file with your back up and hope this time when you press E to the next floor, the next floor/level will have something good for you.

Also it seems like the penis sprites have a black censor bar(the farmer at least) on top of being mosaic'd, was not the case in older version of the game.
attached assembly removes the mosaic btw

Traditional way is using Visual Studio or VS code. But if you don't have them so just edit with notepad

It's dnSpy[ex]. Note that you won't be able to view the definitions in the more recent versions of the game since it uses IL2CPP. You can use a disassembler if you want the implementation

I also wish the shopkepper was a constant in each stage, cause sometimes he doesn't show up, and can't even get his scenes.

Any enemy that has a vaginal animation can impregnate you. A good way to not be bothered with the clothe mechanics is to just remove them : there's a big wardrobe in the church and in safe zones where you can undress.
