I've just tested it again on my end and I cannot reproduce any problems with it. The dialogue is not Japanese.

I got the game from here:

After extracting the game I opened the DMTL patch from here: https://f95zone.to/threads/time-stop-rape-gensokyo-rj198574-nupuryunet.98329/post-6856339
Then dragged the www folder into the game folder and told it to overwrite.

I did not do anything else and it works.

IIRC there are side route to defeat the first boss, if you continue forward the battle will be hard but if you go to the right side the battle will be easy, just search the boss map and you will find another route to the boss fight

I recall getting the translation from this thread, if not this one then check the replies above
and the Japanese game

Description : A short 'visual-novel' (made in RPGM MV) with a series of animated time-stop sex scenes (including sound!), illustrated by Nyuu. Features 7 scenes, set in Gensokyo with characters from Touhou. The game is pretty quick, no gameplay elements, but is one of the better looking time-stop titles out there.

I downloaded the game from kimochi site the link you posted i use winlator to run the game
