i just go to strolling option and click on return after the first time she doesn't accept the job it never shows up again
There's a few endings related to Tomie being pregnant, but also you can keep on to the pregnant expansion as well.

yes! there are actually several possible scenarios if Tomie is pregnant
1. Ending triggers (eg, Tomie dates a person, and that person marries Tomie after she gets pregnant). After the ending, you can continue the game like nothing happened
2. Perform an abortion
3. Continue to the expansion, where Tomie is pregnant and she has to take care of the fetus
