I'd say any of the characters you're most likely to see often, like crew members, it's a travesty looking back at some of the older art, you can see that some of their stuff is good, like Olympia, but then you have all their Kiro busts, but really, look in the gallery after filtering to them and using the full gallery viewer and you will see.
Hmm, so I messed around with the images. Seems like they are individually hard coded which is insane to me considering how many characters and artists there are. If you delete an image, the gallery still knows that an image should be there and is missing. It also won't load any new images or folders even in the 'Anonymous' folder which seemed like a good place to do so.
Fortunately, you can still replace existing images to make changes, but it means an image pack would actually just be an image replacer. On top of that, the HD image pack gets stored in your Roaming folder, so if you downloaded that you need to replace the x1 images in your install folder AND the x2 and x4 in the roaming folder for it to actually replace everything.
Here's proof that it at least works once you jump through all those hoops.
I don't know why it isn't coded to just iterate through every image in the img folder and look for a specific image name. They even have a naming convention:

Hey so, at the beginning of the game i set my character as fertile and etc, but recently, for some reason, my Character became infertile in any way, is there a way to add the Fertility back or ?
Sterilix is a drug that makes you temporarily infertile. If you take multiple at a time you become permanently infertile. As far as I know this is the only way to permanently become infertile after character creation, so that must have been what happened to you.
Luckily there's an item called Fertite+, which removes all infertility/sterility, you can buy it cheap from the Tamani store on Tavros station. I'm not sure if it works on characters that start infertile, but since you started fertile you should be fine.

There's also the Sterillex band you can buy in Tavros that turns you infertile until you remove it.
Okay, so I got Kiro done. I think she looks incredible, but then again I did make it for myself so I'm obviously bias.
Something to keep in mind: Kiro has a ton of breast sizes and I just don't feel like making an image for each one. I feel like if you're downloading this pack you don't want mega sized tits in the first place, so just don't make her tits bigger. Simple as.
She does have varying ball sizes though and I don't think you can control that so I did go through the effort of creating those. They're all pretty reasonable though.
You can also turn her into a bimbo but the provided art for it was atrocious. I went for the simpler approach and just gave her some hair and make-up. She looks kinda like a preppy version of herself, I kinda like it. She also has a hyper variant which I also made, but again, is within reason.
I included the .xcf (Gimp .psd) so you can mix and match things yourself if you want. For some people the hyper size may be more to your liking, idk. There's also a non-erect version but the game doesn't have an image for it which is unfortunate. I think it looks pretty good. Maybe you can find a use for it. Everything should layer on top of each other pretty well especially at the size the game renders them.
So how do you install it?
Well, that's the annoying part. Like I mentioned earlier, it has to replace the current images since you can't add new ones.
First off, make sure you've downloaded the Image Pack already through the in-game main menu. Don't download the files manually. It's the button that unlocks the gallery in the center.
Then, go to

Genuinely looks better, I don't know what's wrong with Adjatha that sometimes they make gold, but then turn out massive piles of garbage, I'm happy technology has come so far these days, and that someone was willing to tardwrangle the machine.
Sick, glad people like it. And yeah, it's definitely nice for someone with no artistic ability like me to be able to still make whatever is in my head. It does still take a lot of time though. Way less than doing it by hand of course, but this took around 4 hours to do. Just tedious is all, especially trying to make small adjustments on the same base image. AI has no concept of image layers or transparency, so there's no such thing as simply drawing on top of something and preserving what is underneath. Have to do it manually instead. Maybe one day.
I'm taking a look at the source. Might be able to monkeypatch the image system with something better.

Please continue the great work when you have time. I used to love this game but this hyper art is death to my eyeballs and lower balls. Would be great if someone could find a way to make custom images easier to add.
It's just using AI to adjust and edit, if speaking of Jake's Kiro image, it's not just lazy AI sloppa ran through a few times and called it good.

Oh sick, what's your process here? I've never seen AI get several images close in a row like that, are you generating a base image and then asking it to modify it? I spent a few days teaching myself Stability Matrix and one or two models, but I couldn't get anything as coherent as this.

So I use completely local generation with Invoke.ai. It's kinda like Photoshop but for AI images. Basically you generate a bunch of images and refine your prompt until you get something you really like, then you start going crazy on manual edits and inpaint.
For instance, in order to remove Kiro's clothing, I literally drew over it manually then had inpaint clean it up. To increase the size of her bits I duplicated the layer, isolated the part, scaled it, then inpainted it at the new size.
It's kinda like handing the pen over to your big brother and asking him to finish your shit drawing for you. Except most of the time your brother wants to draw something completely different from you and you have to beg him to draw what you asked for.
Yeah, a single drag and drop solution would be awesome but they fucked up the code so bad it's kinda crazy. There's so many images and they're all hard coded despite having a clear naming convention. Wild. I tried adding a folder called Jake_James and adding stuff there but it doesn't check at all.
Even when you delete images, it still looks for them in the gallery. It's like a real life gallery where someone went and hung each and every image on the wall manually. I can't imagine doing that and not giving up and automating it.
And then to top it off, the HD Image Pack downloads in your Roaming folder with no option to change the location. That makes your install completely non-portable. Your saves are there too by the way. Even if the game is on a different hard drive, most of the storage is in the Roaming folder. Wild stuff. I wonder if CoC II is any better. Isn't that one on Steam? Are they both on Steam? idk
Something needs to be done with the image pack, it seems to contain only x1 and x2 folders while the game seems to expect x4.
This might be some weirdness with the stub method, unless the backer version just forces x4? The stub one does not download images and Mac users have to use this method since the Mac app has not worked in at least 8 months (probably longer, ever since they decided to move to packaged electron instead of still allowing people to launch via the html).
I could also be missing some setting somewhere.
