No, you're not crazy. The dev is kind of a crazy narcissistic jerk and comes out in a lot of ways (the insane delusional belief he was losing like 90% sales to piracy and spending so much time developing very malicious "DRM").
The difficulty settings are because of "whining." Normal difficulty is really just easy mode. Look at your status effects, "normal" difficulty has a baby icon next to it.
That is, the developer has time for drm, for liking how they prevent him from selling the game, but there is no time to make a game, and not a recap of the previous one with a terrible pace of adding content, raw and usually empty.
But he has time to milk the audience and make the game almost the same as it was with zero progress.
Is that true or am I missing something? After all, the key differences are slightly different enemies, slightly different weapons and so on, a little bit of trifles here and there

Not sure why you're asking the question, that is indeed one of the most common forms of narrative NTR. I.e. the cheating partner who cheats, and we know but the faithful partner doesn't.
It's not even usually a perspective thing, as games based around that very common trope proudly label themselves NTR.
The places where you see the broad hatred of NTR erasing any nuance is stuff like love interest tropes i.e. childhood friend, though IMO the fact those are also narrative tropes (i.e. the childhood friend in love/romance fiction being a love interest), they definitely count when used that way, even if there's no actual relationship yet, and a lot of people share that view too.
