first 2 pictures show witch camp and the door + leaver you need to reach, the rest show entrance and how to reach the leaver via the shortest route, if you have not killed the monsters shooting cannon balls you might have to go through the sewers to reach the leaver( if you have opend the door from the other side in east library you can use that route to backtrack through, there are 3 ways to reach it if you have unlocked the shortcuts but i dont see you not pulling the leaver when walking past it so in that case you would already have opened the door because the only way i know is through sewers if no shortcuts have been opened, unless you walk through cannon ball explosions that is
edit: if anyone is wondering this is how and where to open the door to the first blasting bomb for one of the cracked walls on layer 1,
on picture 4 if you have to go through sewers you come out from the north but you cant see the stairs in my print screen

Where do you find the underground city? I assume it's the big place in the 3rd floor forest but couldn't find a way to get in

nvm, found it thanks to one comment here. so you need to go through the leech traps, and there's a room at the end. it brings you back to the tutorial place. there's another poster that mapped out the whole place, but he missed this room.

you need to use peter the fuck-killers rout but instead of pushing the statue on the floor panel to open the hidden door you go south pull leaver kill red bastard then head down follow path until a T section take west walk through a bunch of leach traps tada your now back in tutorial area....
but good luck with that you will die halfway through if you dont know how to counter the leach traps
