You'll need to have sex with the vagrants in the tent, to get that scene, you'll have to have had 50 partners and get condoms from the love hotel.
Titles do not change how the character acts. It's really more of a string of text in the menu to let the player roleplay the character as they wish. It's kind of like what people do in MMORPGs by putting a title on their character for other players to see, but since you're the only player it is that much more pointless. You do it just for your own amusement.
Rei got the impregnated status from the second vagrant event, is there a way to progress companion pregnancy or is it just there for flavor?
