Not so easy to answer, the open relationship thing only started to get some ground last update but I think that it is fair to say that Miru is not a cuckqueen that will be glad in let William fuck everyone while she watch, even with the NTR turned off I don't think she will be pleased if you fuck someone on her "big no,no" list. If you are with the NTR on, it will go both ways and Miru will fuck other dudes and - this is my speculation- will require some "management" as is a situation that can spiral out of the MC control.

You've had a coupl of fantastic answers, I'll add my two cents: I believe Ocean is simply offering people the option to avoid content that isn't appealing to them. I still think William can cheat regardless of NTR on or off, but most likely with it off, the LI's will stay faithful even even if William pisses them off. Most likely they'll get revenge in different ways. I'm not someone who generally likes NTR, but I'll leave it on, simply for the threat of what can happen if I screw up.
