If answering the question from the description, you should know that the man and woman in that image are not the husband and wife (the two MCs), which I think led to the confusion in the response you got from it.

Well, I did not play this game (yet), so I was just looking at the images and reading the game description.

well u have the choice and u have 4 routes.When u launch the game make sure to read the guide so u can know what every variable do and what is required to be on some route.

You didn't miss anything. That scene was setup for future scenes and to start communication between the couple.

plus u can't add flashing,groping in every scenes it can slow down the game.The dev are smart...

Because nothing new happened and there was no development in Sarah or the MC, the update is just about Dr. Eva only.
