First they aren't "afraid" they either dislike it or hate it and so when they see a tag of something they don't like there's an instant disinclination to play that game. Some people will bite the bullet and see if its avoidable, and some just wont play at all.

People use porn games to self insert. It's not the same as a video because you influence what happens in the game. You are in control and people don't like that despite being in control things are out of their hands. They want the game to focus on them and their wants. They want all the girls. That's fine. People should be able to self insert if they want to. I don't care for harems though.
