2.All Menu's and every choice is translated as well .
3.The game itself do not have patch applied you'll have to apply the patch yourself.

So is the entire game the male POV? For tentacle scenes, does he just do the "stand off camera watching" thing? And how does the pregnancy tag work?

yup The game is on Male POV and for tentacles as you said the "stand off camera" is right term, for pregnancy is as an ending , there is an ending pregnancy sex scene.

I always found it awkward when devs insert tentacle scenes in a fully male POV game, like he's being NTRed by...tentacles and just standing there the whole time?

Pretty sure the female dialogue in the sex scenes isn't suppose to be gone. Removing the patch fixes the problem.

It's written in the readme.txt of the save data. If that doesn't works try onedrive's document folder.
