Do you want text that sticks to OG japanese 100%? Then play MTL, it's faithful to a fault and decent enough for what it is.
Do you want text without weird grammar mistakes or some very awkward translation occasionally? Then play the official release, it isn't as faithful as the MTL but it flows a lot better imo, also stuff like sex scenes and item/UI descriptions really benefit from it.

Edit: I'm not sure if there's anything new to this version other than QoL improvements, I think they've moved on from summer to winter memories and are modifying that instead

Yes, The older censored version would be on the bottom under old mtl or whatever its under. Kagura version is pretty good.
