Where does this trigger?

Sales area - while working on leads?
Break area - while chatting with coworkers?
Bathroom - after checking make-up?
Working as secretary - while doing the wordle mini game?

On some play-thoughs I expected there to be more consequences after enraging all the coworkers.

Not 100% sure, but I believe any approach during sales job(not secretary) can cause the event as long as you and the npc meet the requirements. The ones I remember is you have 6 office rep and npc has <0 respect for you.

Edit: Requirements are:
1. Player is female
2. Work hours left >2 and <7
3. Office rep >4
4. Not Non-Con disabled
5. No other approach that day
6. NPC respect <0
7. In your office
