The womb tattoo and piercings appear to be optional. You go to a sex shop, ask for it, pay 1000g, and must have sufficient level of affection first before they do it.
It does unlock additional scenes, which in turn unlock new combat skills for the girls. But you don't really need every single skill.

You go the folder it says, it should have a slightly misspelled file. rename it to match what it says on the screen. if you can't find one then get a different costume and copy it and rename the copy to match

Can you tell me if the tentacles and multiple penetration are avoidable?

No idea. I have not reached there yet.
Incidentally, so far the peeing I was able to see was avoidable. so that is 3/3 things being avoidable so far, which is amazing for a japanese game
Oh, there are female condoms too so impregnation is avoidable as well.

I'm not going to bother trying this until it gets a full translation but I gotta ask because I just find it to be a really strange sort of tradition but does this game also take place in an alt-universe Italy like all the other fantasy style games this circle has made have?

But only a controlled one that could be avoided and puppeting is also not bad, I never understood those who put themselves in the place of gg, the game is what the author intended, that is, following a certain scenario that the player chooses, in life everything is different, we ourselves choose our future as much as we can influence it of course.

Very low quality even for the usual MTL, a good 10% of the game was left untranslated and quite a few bugs (probably not introduced by the translation patch, but i can't be sure as i haven't played the original game). Would wait for a more proper translation or at least a fix to the current translation.

Version 1.10, so is this a final released version 1.0 (complete main story)+ additional content hence 1.10, or one of those games that first early released starts with 1.0?

This is an absolute masterpiece. I played the first half hour and then uninstalled so I can finish the game once it's been properly translated
