It's an adult game, not a porn game.
People get murdered. Stabbed. Killed. Shot. Zombies get slaughtered wholesale. There's IS a hand-job, a short lesbian scene, some naked women in showers and so on.
But it isn't a PORN game.
The focus isn't on characters having sex, but on a secret society of warrior nuns and you saving the world.
I understand it may or may not be everyone's cup of tea, so enjoy it for what it is.

Yeah that is what I don't get I swore her to Sororo Misericordae, so she could stay and did not choose to send her away.
now it is as if I had send her to the hospital and not kept her.

I, too, swore her into the Sisterhood, and got the hospital route. Used the Console option and amended "sadie_misericordae" variable to True and replayed that section and got the Ssterhood route.
Don't kwo what happened the initial variable in the save files (or persistent file).
