You can use keyboard just fine. I don't play with a controller.
There are flaws in the game certainly, but the controller thing isn't really an issue.

There are 4 camps that are optional, just killing a few soldiers. Then some dungeons that are optional too I think.
But the 3 main dungeons that appear in yellow i think, those are quests to rescue the girl kidnapped, you better kill all the mobs and then move the girl to the end, because she can be killed and you have to start again.
Then there is a final dungeon in the left part and when you finish that dungeon you rescue the magician and the game is over as far as I know because those last 2 3 versions are just the same, I dont know if this last version adds more dungeons...

As hot as the queen is if she loses or gets captures the enemies should be tryna bang her nobody could resist tits like those haha. That is a good incentive to not die as well cause as of right now you just respawn and get to start over without any real consequences.
The sex scenes are softcore, for the female character you have to buy and wear the chain harness when your in town, and interact with one of the males in the bar. There will be two options, one for a blowjob and two for a spitroast orgy. All scenes are softcore nude. You can also repeat the scenes once you leave town
