Like i said before, some people may want (and always do) to completely become character they play, associate themselves with him\her (it's protagonist after all), to greatly increase enjoyment of game, like "you're living in game's world". It's not only about porn games, it works in RPG like Elder Scrolls and Fallout, visual novels etc. And some people don't want to associate with their character, and see this just as fiction. Only one of those works for each player. It's not about knowledge of this genres. Even whole NTR as genre basically have one large goal - to cause jealousy from us. Not just for show.

It's more about author's choice and his skill as writer (POV can help of course). Or, if he\she wants us to associate with protagonist or not. There is stories, where we see all situations from third invisible side. But, you can sorta associate yourself even with evil or psycho protagonist. It's fantasy after all. In some japanese visual novels you can really immerse yourself in story and associate with protag very well, exactly because of writer's great skill.
Yeah, i know about netorare very well actually, and member of both this forums. I said "basically". Jealousy is just one of the goals there.
