She is not "mentally damaged". Traumas don't necessarily stop people from being able to live their lives. Also the game heavily implies that her had been in a sexual relationship with her friend Jada via all the dialogues and the butt plug she received as a present.
About Jill's "stepmother" (since she never married the protagonist's dad) you can choose if you want to take advantage of her state after taking her medicine or not.

Ellie went missing a few months after MC ran away and turned up a few days later unable to speak. She's traumatised not mentally handicapped.
It's all explained in the story.
No she wasn't raped that was cleared. It's hinted she saw something she shouldn't have and it left her traumatised.

Regarding Rachel, MC used to hate her, he/she basically still hates her. He struggles with himself all the time when it comes to Rachel, he doesn't trust her, and after all he went through, the little things he does, he sees as a payback for all the bad things Rachel did to him.
There's a little problem here though: Initially the things Rachel did to him where meant to be a little bit more than just verbal abuse, but after ther initial testings (before the first release) I got some of feedback about that it's too much, so I cut most of that.
It was the right decision imo, nobody with the right mind want's to see (read about) that in a porn game after all, but of course you don't get the full picture now of what MC was going through in his/her childhood/teens.
Btw you are still missing a few things regarding the story, so I'd recommend to just play through to the end, and a few things might become clearer (if you didn't choose to ignore some paths).
