"Going back" is just to let the player know that new H-content opened up in the earlier towns. But to advance the story, you should go west after getting the fast travel item.
Can someone tell what to do about Village Ruins? I was able to acess two houses with puzzles but I cant really progress further. After which point of the story I can go back and finish this side quest?
U get it from an npc in an Inn that you visit during the main story while the Samurai's younger brother is following you.

You need to unlock ability to break brown stones, move gray ones and to swim to access all houses. Just advance main story further.
Last two houses can be reached from sand caves at route 32. You will have to go underground and then swim.

The Miasma Cave (Evening cave) is ridiculous.
Entering it and clearing the miasma leads to a closed off windy map.
There is an exit to the north blocked by a lake too!
What to do to progress?

That's not how you progress. It loops back to the starting map. You go south in the three way fork after the tower
If you can't progress, you have to pass by the ruined village first
