Just so you know, the love route ends somewhere around chapter 5. After that we have basically the sharing and cheating routes only.
However, to answer your specific question, thus far (Ch8) the MC only ever gets to be with fiance (girlfriend).

Yeah, this game is (now) mostly about the fiance (girlfriend) getting it on with others.
That scene was cut from the game. You'll need to find a very old release (0.3 maybe?) to see it.

I am just curious....do you know why?
It was cut because story-wise it didn't work. The scene that led to the hospital was problematic and The Typist didn't like it.
It's a hot scene though, so maybe The Typist will find a way to use that art in a future release.
