The biggest consequence is that you miss out on 1 girl to have a sexual act with if you go for Bella (you can get Rose with Derek or Ashley with Sage). That might be important if you want to have the maximum amount of girls possible in your stat screen.

The only reason to choose Derek is to avoid certain temptations with LIs (or to have avoided them before) to then give in to a SG who practically disappears afterwards is not very smartIt all depends on if you think Rose is worth the toe in your mouth over Ashley.
