I dl'ed and opened it, found out the translation with the L button only works for about 60% of the text the rest stays in russian. I hope someone can fix that someday please.
That's not an issue that has anything to do with the game. It has full (sometimes noticeably poor) translation from its original Russian, to English as standard.
You played a mod, a fan made addition that's written in Russian, with probably little-to-no English translation. This is not the place to discuss the mod. That's what a thread about the mod is for, and if it's the so-called "v0-14" mod, then I don't think it's here.

For example, it would be enough here to make a prologue (as we have seen in some sagas of films) Big Brother - The Beginning in which it would be explained that:
1) Ann and Kira's parents could not have children, so the 2 sisters were adopted at birth
2) Ann could not have children or she wanted to continue in her parents' footsteps and so Liza, Alice and Max were also adopted.
Thus, there is no more consanguinity between any of the characters, the rules enacted by Patreon are respected, and the game can continue to evolve without having anything to change to the existing game.

Been tried, and those who did, still had to change to having any of the characters involved in sexual relations with anyone else in the same household to be absolutely unrelated by blood, marriage, adoption.
Considering that Patreon's "rules" i.e. their terms of service, and what they practice are not the same thing, and it's actually whatever they feel like at the time based on ideology and/or pressure from Paypal (that's where the crackdown originated) and whomever else controls Patreon's wallets.
