Work on saved game, don't need to restart everytime, just make sure you save your preset before launching the game, to load the preset on you and save it for good visit

in the alchemy, talk to her, make her change your appearence, load your preset look that you created on the variant cheat and when you are done, your new look will appear on you, once your game is save at least once, you should have your new look everytime, if not, visit again Camilla.
The Variant cheat can also make you a Nephelym without the spirit mod, you simply load a variant, like a fox, don't mod it, copy it's appearence, then load your character preset save while you're still in variant cheat, press to load the look on yourself and you'll be a fox, save your look, visit camilla to make it load on you, save your game and it should be permanant, if not, Camilla will fix it again, might be annoying to visit her all the time if it doesn't save on you, but now you know that you can have any look you want without starting a new game everytime if that was your question.
