While I believe a milf can and should look both young and hot, she should still look old enough to be a mother. Giving birth at 18 would create a 36 year old mother with an 18 year old child. 36 is not old for a woman. At that point they may not have even reached their prime. I've given a good deal of thought to this (ahem). First of all -

he'd be both a brother and a father

He would be a half-brother because they would have different fathers.

As to, whether he should call the baby his child or brother/sister, I think it depends on how he sees his mother. If he still sees his mother as his mom, it would be weird to call her baby his son/daughter. I think brother/sister from another father (i.e. himself) is more appropriate in this case. On the other hand, if he starts to see his mother as his lover/wife/partner, he could call the baby his son/daughter.
