I replayed the quest today. Looks like you need to talk to Tefahra. If you play along you get to choose which drink to order. There are three options: Spiced Wine, Narthossi Stouts & Blood-Reed Tonic.
You should find Tefahra in the tavern, after you dealt with the orgy up north and went back to her now empty hideout in the ruins.

I stopped the orgy (by sleeping with the priestess) and went back to the hideout where it gave me the "quest" to get the tonic but when I talk to Tefahra in the tavern she still says "We'll talk more after you've done what I asked" and I can't progress.
[Edit]I replayed the orgy part and killed the Anubites, now I can progress and order the blood-reed tonic.
[Edit 2]Replayed from the same save again, slept with the priestess and now I can order the blood-reed tonic as well. Must have been a strange bug.

There is also an interaction with the Cat Lady in front of the pool, close to the villa. I think it's limited to human cocks though.
They set exactly what they are supposed to: your ability to conceive offsprings.
As regards "gender preferences", it depends on whether you have a cock or not, then certain NPCs will react accordingly.

The patch notes say that they fixed the loading of outfits, but the "bedlah" outfit is still invisible (at least the stockings for sure). I'm amazed at these developers.
The problem with getting a "completed" save is this game with not that many scenes to begin with has a bunch of non repeatable scenes, this game really needs a gallery or to just bite the bullet and make every scene repeatable no matter how much that doesn't make sense.

They actually have roughly the same number of quests and sex scenes, but UE5 spreads those out over a much, much larger map. UE4 definitely has a lot more in the way of random forms, equipment, and "equipment".
This game desperately needs a gamedesigner with sanity parameter > 0. And maybe some occasional violent beatings with a keyboard to remind some of them clowns they have to work for money, not just drop random shit hoping it will assemle itself into a complete game.
When I see a mess like this I always remember, that Shell's gamedesign book have paperback edition - so it can be used to physically educate stupid fucks to some Intelligence ++.
