And thanks as well for reporting the missing step -- I will have to double-check though, because if I remember right, if you have both the herb and the dead frog in your inventory, you'll only have to give Matilda one of those items (either one would work) and the game just gives her both ingredients. But again, I'll have to double-check, and if this step is in fact missing in my Walkthrough, I will of course add it as soon as possible!
// EDIT: I did a quick check: there are three items we're probably talking about:
Herb,
Bluebells, and a
Dead Frog.
- at the beginning of Chapter V (page 12 of my Walkthrough) you have Herb and Bluebellsin your inventory
- you'll only need to give Matilda the Herb
- you can give her the Bluebells as well, but it's not needed at this point
- in the middle of Chapter V (bottom of page 13 of my Walkthrough) you have Bluebells and a Dead Frog in your inventory
- you'll only need to give Matilda one of those items, and the game will give her both, so no extra step is needed here, and it doesn't matter which item you choose
Maybe you've given Matilda the Bluebells instead of the Herb at the beginning of Chapter V? Because then you'll have to give her the Herb as well as the Dead Frog later, so that's probably why you're thinking a step is missing...?! I'm not sure, just guessing.
Anyways, glad it still worked for you, and keep posting if you find more errors/mistakes in my Walkthrough!

Because the sequencing of actions in the game is very flexible there's the opportunity to do things later. However the game also tells you that some things cannot be done later on, otherwise you're going to be unable to achieve the side quests!
As a result, I now remember that I indeed gave Matilda the Bluebells earlier, because I was worried that later it would be too late! The game tells you, at the end of Chapter IV, when you go to the Bedroom in the Inn to have sex with Ashley, that: "Warning! Triggering this scene will end the Chapter. Proceed?". At this point, I was afraid the guide might be wrong or hadn't been updated correctly for the DLC (sorry!

), so to be safe, I decided it was better to give her the Bluebells right away. At that point I searched the guide for the terms "Herb Description", "Herb", because again, the game warns you about missable quests, but the guide hadn't mentioned using that item yet. I started making multiple saves because I wasn't sure the guide was correct.
The fact that giving the frog was completely missing as a step also worried me. I haven't tested this, but I believe you when you say that the step is not necessary, but from my perspective, the perspective of a player that doesn't know the game as well as you, I thought it was missing.
The same is true for telling Liam his identity. Again, because of the "warning message" at the end of Chapter IV, I had to search the guide to see if that step was written later on. I understand that it's optional because of the multiple endings, but I had to search the guide for that particular step, still because of being afraid to miss it.
If I could offer a suggestion, it would be to add a reassuring message about the side quests at the end of Chapter IV. Imagine the concern of a player like me, holding on to: Herb Recipe, Bluebells and the Penknife, when the game gives you a warning before the transition to Chapter V

. If the guide said something like: "All side quests will be completed in next Chapter", it would give players peace of mind.
The same goes for Liam. Right after you discover his identity in the book, it would be great if the guide mentioned something like: "You will have the option to reveal Liam's identity to him in Chapter VI, depending on your chosen ending."
