the story doesn't have a will of it's own, it will not "demand" anything and nothing will "fit" or not unless you want it to. You created it and every aspect of it. Period. If you don't like bestiality then that will never show up, even if a character has a dog because if you don't want that content then it wouldn't even enter your mind to have that as a plot line. If you rule out incest, pregnancy, necrophilia, or scat, those will never show up, and if you like them and want them, then they will be part of the story and you will make them fit because you are in control. You can't blame the story for choices you make, you created and control every aspect of it. You wouldn't add NTR if you didn't like it at least a little bit.
All having this element in the story is going to do is drive away a bunch of people, and it will not even bring people in, because you've already said it won't be a focus. So, either the NTR fans will get board and leave too, or this will become a lie and you'll end up adding more and more NTR to try to attracted those fans since you drove everyone else away already.
And you won't be doing this with other fetishes I bet. And by the way, optional paths and side content is a false promise, it never works for many reasons but let me give you an example using another fetish:

Now, maybe Ryan is correct here and you are still "confused" by what NTR is and means. It happens since the site definition isn't the same as the real definition from Japan, and it isn't the commonly used way of thinking about either, and some people will use NTR for netori as well at first until they see that the site doesn't group them together. So I'll ask a similar question in another way and see if we can get some clarity:
Will the Main Character be the only one having sex, on or off screen, with a girl who is or can be a love interest after the story begins or will there be times when a current or possible love interests is shown or clearly stated to have been fucking other guys or girls during the course of the story?
This is the crux of the matter, people who don't like NTR hate seeing or even hearing about girls that the Mc can ever be involved with being shown, or hearing about, having sex with other people. Any sex that doesn't directly involve the Main Characters is a no go for them and will make them not want to play the game even if it is an optional scene or theme with a single girl, the same way that many players will never play a game with even a single scene of bestiality, scat, or necrophilia. People who don't like NTR see it as just as gross (to them) and off putting as those tags and will not want to even risk a moment of being exposed to the content even by accident or by hearing about it.
Anyway, just my two cents on the matter, or maybe two dollars considering how long it was,
