I think there's a walkthrough option somewhere in the settings if the numbers aren't there by default.
It'll be such as.
Option1: Plug both earplugs into ear
Option 2: Leave earplugs out of ear (1)
The (1) or (2) etc indicates in what order you should pick the options.
If you're looking to clap the sis or mom then it ain't there yet.
Maybe next update or the update after the next.

The update schedule went from approximately 3 to 4 months per full release (not counting hotfixes).
You can see for yourself on the itch.io page, there should be a timeline with dates for every release post.

An NTR route for the mother seems to be planned. That is just based on the fact that there is an explicit "Non-NTR route".
Right now, there isn't actual NTR. It's just the parents having sex, and this allows the MC to... establish the first sexual contact with her.
