One is VN, other RPG. RPG beats VN any day, regardless of the type of fetish. It's basically "game" vs "story".
But would you fully play a game with NTR where it is avoidable (multiple endings), BUT, in order to get ALL achievements/get ENTIRE gallery you have to endure it? Or would you just play non-NTR part, without getting all achievements/gallery?
I guess this is similar question for regular games: if you like playing only "good" characters, would you play "new game+" making mc "evil" just to get all achievements, or you don't play?
What is stronger: your "uncomfortability" to play a certain unlikable character or curiosity "what is going to happen on this road"?
Leaving aside the game vs story debate, there are many stories I would play and it would never be like dark hero party.
Related to your question my uncomfortable element comes basically from the following:
1º Pathetic MC
2º Evil bastards as antagonists
Something that tends to happen in NTR games is that those 2 elements that I personally dislike quite hard are there, you can't find an antagonist that is overall a good person but lustful or perhaps someone that would only try to seduce the love interest if you had some sort of weird relationship like slave/master and stuff, or perhaps that they are actually totally good people but that fell under a curse.
Also the MC doesn't help since it's either a full pathetic mess or an absolute cuck who ends up enjoying his loved one having sex.
If you took out those 2 elements you will notice that the NTR content would be quite light hearted and not too impactful, which would be perfectly fine for many games to have as a gameplay element to counter. This is the thing, I can understand the value of risk of NTR, but you have to be mindful with your audience related to how hard it goes.
Let me go into games with NTR that I played:
-Leane of legitimate crown: I did play it and overall I liked the gameplay, NTR was kinda on the hard side but I was a bit forgiving since I can avoid it, but the MC was an absolute dislike for me, since he isn't rewarded good enough for playing good, you are locked to 1 romance per playthrough and only one scene + a light ending.
Would I play it for the NTR scenes? No, my curiosity didn't get into it since I'm not into a bunch of bastards having sex with girls.
-A struggle with sin: Played it, liked the gameplay, but it's the opposite of what leane of legitimate crown is, NTR feels very easy to avoid, to the point that feels more like how matching characters in fire emblem works. Do I play NTR scenes in it? to be fair there are many NTR scenes that are quite light so I don't care at all, sometimes for characters that I don't like a lot will just let them have a boyfriend or whatever, I'm not someone that requires a harem of 20 girls, most of the times I have a clear preference for certain girls and the rest I will do whatever I want at the moment.
