What you found is all there is. To me it seemed like the dev wanted to add more but something happened. Good news is it's getting fixes and minor updates so maybe he'll add more levels in a dlc or something.

UR CT break in the HP pointer after clear one stage/zone, a;though when i get dmged it seems 2 fixed its self. X3

You may only hold one. If you pick up another, item you were holding will be gone. Medkit you can buy and switch to at level select can use itself automatically when you drop to 0 HP iirc, I recommend.

Is the game buggy or I don't understand how to save in this game? Every time I start the games it's from scratch, having the intro and tutorial each time I start the game is annoying
Have you extracted the game from the .zip/.7z archive? That's the most common reason why people seemingly can't save in games.
So the "final version" of the game crashes the instant the actual gameplay starts, and half the UI isn't translated into english even with the english option selected. Not good.

I went and bought it on Steam because I realized that definitely I got $10 worth of entertainment out of it. You're not missing out on too much.
The updates fixed most of the crashes and gives the shopkeeper a scene (unvoiced), but you can't replay it from the diary. Mushroom non-futa still crashes.
For a game whose H implementation is just "get knocked down, watch animation; lose, see scene", I find it pretty inoffensive - it's quick and easy to play through and the action gameplay isn't horribly janky like so many. I like the variety of weapons, equipment and upgrades, though a couple times I did end up with totally shit builds. But all around it's pretty well designed, and has basically no padding with plot, or gameplay sequences with no H elements.

it's not a mechanic, just a couple of CG sets from getting bad ends with certain enemies.
that said, game is fun.
