Nancy, Penelope and Dalia could EASILY be family (Nancy the young aunt who babysat him and the other two being cousins). I'm playing it just now, because I'm still crying from this not having a patch, so didn't try it when 0.1 came out. At the moment, can't say with certainty whether it would be needed or not, not sure if things like the MC at school saying "That's just Dalia. You grew up together! She's practically your sister!" is just simple bait and teasing and whether a patch you change anything significantly. You see, Once was by default step and the patch just removed that fugly word and the game continued as it was (dunno how the heck Patreon didn't do anything about that). Unsure how this would fit here, are a lot of changes required. I am all for a patch, would be beggin', BEGGING for it, as long as it as a "totally non-official, Totally not Caribdis, totally not affiliated with Caribdis" patch, instead of a random schmuck going "Imma patch this"
Could easily be, but are not, the story isn't set up around it and even the developer has said incest will not be given any thought. A patch is not needed because there is no incest to patch, the developer specifically said there wouldn't be any. "You grew up together" is not an indicator of family, it can be, but it can also mean close neighbors and childhood friends of other sorts that grew up together. Patreon didn't do anything about "Once In A Lifetime" because they couldn't, there was no offense by the developer that they could prove and they could not strike Caribdis because of that. However, that game was specifically built around the sisters being sisters, step or otherwise, while "Eternum" is not.
Honestly, i'm all for forbidden romances...but do we really need incest(and incest patches)? i mean the "from childhood friends to couple" is a cute vanilla trope if written properly(which Caribdis is a master of it, he already proved it to us)
i mean in the end, a sister(or a brother), isn't a childhood friend who simply happen to be blood-related to you?
For example in MyNewFamily, the incest patch is mandatory, the whole thing is about a reunited family who fell in love with the son/brother who they never met until he was 18-20 years old, remove the incest...and you lose the entire premise of the game.

We don't need it, especially when the game was not built around any sort of family, not even step. Incest games are specifically built with that content in mind, it doesn't work without feeling artificial and forced otherwise. Your example, "My New Family," is a prime example of one that was made to serve the kink and it works naturally and without being forced because of it. "Once In A Lifetime" was also built with the capability to patch out the step relation in mind. "Eternum" was built with no consideration toward any sort of incest, so out of the three I have named, this game would be the only one that would feel forced and unnatural.

Yes they are, from the very beginning I might add. If incest is in the plan, then there are provisions made within the story for it to be added if it can't be added from the start. There are no such provisions here and anyone who patches it would have to go out of their way to make things fit incest because it was never meant to be here.

I mean, the plot of The Inn, without the incest patch, is about two foster-parents(or orphanage managers) adopting 3 kids(Luana, Val and MC) and growing together for a long while(and developing a form of love for each other), until the male-manager/foster-father kicks out MC...for reasons.
