The other photo from the war scene has got the eyepatch correct. This image is flipped, yes :/I think it's mirrored, if you look the uncovered eye is green.

Agreed, it's definitely flipped it's nothing serious wether it's intentional or happened casually, in the game everything will be ok I think.

It's obviously toothpaste. Otherwise why is some of it in her mouth? It's probable that she was brushing her teeth but used way too much and some of it spilled. But I wonder how it got above her lips and on her cheeks? It looks almost like the tube exploded on her!

Yeah. I'll bet it was flipped when making the promo photo because having the flipped version looked better there compared to the real version in-game. Yep, happened to me as well when I was working in an advertising agency, sometimes it was better for the composition while sometimes we had so much pressure and short deadlines that errors like this occurred.
More time than I like to remember....
