Not sure if there is a Sim that runs on Ren'Py. I think thats why QSP is used.

Depends on which version you want to play.
0.8 requires nothing but itself so you can unzip it and play. Severely cut down and limited version of previous.
0.52 is full game
0.6 is the update files only to update 0.52 (won't run without 0.52)
0.7 is the update files only to update 0.6 (won't run without 0.52 and won't play right without 0.6 also)

I haven't seen a dating sim in Renpy so far. Maybe possible to do but i did not find any game that does what this one does in Renpy.
Though i downloaded myself Renpy to play a little with it. Seems great since its opensource.
You forgot the 0.71 version that is on Patreon. So in a sense, its better to be on the old version upto 0.71 and play.
Presonally i like the game. I was torn between the Aboutit and this one, but this one has its own renders and animations which makes it so much more better. Even if it has not as much options.

There is something that made me thinking.
How someone would end up in prostitution and or use drugs. I haven't explored really interactions with other NPC so far. But it seems that there is no one that introduces you to the drug itself. Like to make you use it.
The alcohol path makes sense if you have no money and meet buddies that introduce you to alcohol when you eventually will be addicted to it.
So i think there is potential to make it more realistic in how you get introduced in taking drugs in the first place. Also prostitution itself.
So far, i just went straight with prostitution but that is not how it would normally happen.
The same goes for the pimp. In reality you would be blackmailed or forced to do things like that. Not sure if there is a path that makes you do it.

Yeah I did forget to mention the 0.71 update. I'm not a fan of the new 0.8 because everything that was removed. I just suggested the 0.8 to that person because it's size would make it easier to download and get the feel of the game for those that haven't tried it.
The game really isn't too much different in style than games like Apartment 69, Milfy City, or Mythic Manor. I would like the interactivity of a Renpy engine over QSP is all. If I had my choice of it being made into another engine specifically it would be Unity, but it's just my preference for these type of games.

Well it seems that QSP and RenPy are opensource if i checked correctly. So no cost for the developers.
Py is always good i think.
As for the game itself. I think if a game can be written to never end, i do not care really on what it runs. RenPy is surely nicer to look at. Though i am speaking purely as a user. Perhaps QSP is easier to do?
Your game suggestion i need to follow up. As long as they female protagonist.
I installed FemLife without 0.8 at the moment but noticed that i miss at least two graphics. Mm.. Perhaps i need to reinstall it.
If i remember correctly 0.71 had auto makeup and razors removed. So less pain to look pretty.
p.s. what does sandboxing really mean in regards to renpy? Endless?

Definitely. I was thinking more of other things like human trafficking or shady loans. Of course the game has a different outset. So the human trafficking part does not roll with that. But loan could be something that would make sense. Low on money, need of money, taking shady deals. But that was just my wish than anything else.
