I seem to remember that it's only available after beating the game once. I don't remember if there's any other conditions, but when I played the MTL, I was able to do it after finishing with a love relationship with Lisa.

I don't think it is, at least from what I can remember. You begin with your girls getting raped when defeated/assaulted during battle, which evolves into NTR through story progression.

Yeah, this is pretty much it, you only get NTR'd through rape, although the actual NTR route doesn't actually start until 2/3 through the game with specific battle losses.
Yes there are five endings. One love and one NTR ending for each heroine and a Harem ending that is only available after you started NG+.

i believe that's the japanese version, the latest english version is the v1.03, the official website has the uncensored patch only for the v1.03 and i think the japanese version itself doesn't have any new content but rather just a few bug issues that have been patched or something like that.
